Tax Credit Donation

FAQ:

1.  What amount should be donated to Mountain Pointe Girl's Volleyball?

A donation of any denomination will be greatly appreciated.  $25, $50, $100...Amounts up to $200 per individual or $400 per married couple are eligible for tax credits. The Arizona Department of Revenue information on School Tax Credit is found here.

 

2.  Why is it necessary to send a copy of the receipt to your coach?

In the past, player's families have made tax credit donations to the team, but bookkeeping errors in the District Office sent the funds to other Mountain Pointe activities and, on occasion, even to other schools!  With the receipts and/or copies of donation forms, your coach will be able to ensure the donations are received into the Mountain Pointe Girls Volleyball Account!

 

How to Donate:

Donations may be made online or through the mail.   

 

For Online Option (credit card only):

  1. Click here to begin the online donation process - a new page will appear.

  2. Creat a ParentVue account or log in to existing - another new page will appear.

  3. Select Items at all Schools - a third new page will appear.

  4. Select School Type-Find MOUNTAIN POINTE HIGH in the Select School Menu.

  5. Select-Tax Credit Donations (NOT $50 ECA FEES)

  6. Find Girls Volleyball - 86-6000536. The list is alphabetical. Girls Volleyball is halfway down the page.

  7. Select Check Out-Continue with your daughter's name and donation amount.

  8. Follow the remaining instructions as they appear on the form.

  9. Print two receipts. One for your tax file and one for your daughter to deliver to her coach.

 

For Hard Copy Form (check or money order):

  1. It may help to print this page first before beginning to have instructions handy.

  2. Click on this link: Mountain Pointe Tax Credit Form PDF. - a Tax Credit Donation Form will appear.

  3. Print the form.

  4. Once printed, complete the upper Contributor Information section of the form with name, address, and phone.

  5. Complete the Method Of Payment section of the form by checking a check or money order and indicating the donation dollar amount in the blank.

  6. There is an "X" marked in the upper left of the Contribution Designation section indicating that the donation is to designated for a specific school (Mountain Point High).

  7. Place the donation dollar amount in the highlighted blank for Mountain Pointe High.

  8. In the lower section entitled Contribution Designation, another "X" has been marked for Specific Eligible Activity indicating that the donation is to be designated for a specific activity at Mountain Pointe High School. As that activity is the Girl's Volleyball Team, "Mountain Pointe High School Girl's Volleyball" has been typed into the blank.

  9. Place the donation dollar amount in the highlighted blank below the "X" (yes, this makes three times the same donation dollar amount must be added!)

  10. Make a copy of the form.

  11. Send the completed original form to the address at the top of the form. (Enclose check or money order.)

  12. Send a copy of the completed form with your daughter to deliver to her coach.
